No audio on speaker of remote SMT-5210

I have a customer with a 7200s V4.99 with an OAS card V2.10.  They have a remote user with an SMT-5210 phone v1.54 that is getting no audio on the speaker.  Her phone will not ring.  Her phone works fine on the handset and headset.  I set up another phone in my shop hooked to their system, tested it and sent it to her and the same problem.  Other remote users are working well.  Does anyone know if the speaker phone uses different ports then the handset?  I had her forward port 9000-9001 UDP through her router to the phones IP but no change.

Does she get dial tone when pressing the speaker key? 
The phone doesnt use different ports for speaker vs handset.
It does use different ports for TDM to IP vs IP to IP.

Default ports:
KSU(nonIP) to IP:30000-3xxxx (depends on how many MGI channels you have licensed or OAS cards installed)
IP to IP: 40000-4xxxx
RTG: 45000-45xxx

If her phone doesnt even ring, possible the speaker is toast.

She gets no dial tone on the speaker.  This is the second phone I have sent her and I tested it in my office off the customers system and it worked fine in my office so I know it has to be on her end.  I just have to figure out what could cause this in her router.  Usually I see all or nothing with audio issues.  This one is different for me since it is only the speaker and ringer that are not working.

Are these ports forwarded to the OAS card IP address in the main office:
IP to IP: 40000-40031
RTG: 45000-45031
Port numbers could be different if you also have MGI license active in the MP20s.  The ports listed above assume you don't have any MGI licenses.

We do not have any MGI licenses but I know it is using MPS ports off the processor.  We had to change the OAS ports to 41000 and 30100.
 I haven't forwarded to 45000 ports so I will do that and then test.  Thanks.

I dont think this will help but you might also turn MPS off.
