7200s help needed

ok so about 5 months ago i installed a 7200s for one of my customers. the system worked fine for the first 4 months or so and then they stated having this wired issue where on some calls not all of them,  in network or incoming via the pri trunk. what it does it some times when you call an extension they pick up and sometimes there phone keeps ringing the caller can hear the called but the called can not hear them all they get is a ring tone, the same thing happens if a incoming call though the pri to the AA. i have checked the logs there are no errors other then when the pri went down once and a few system restarts. the call logs form the AA  looks like its routing the call the way they should and the system shows that the line that is getting the ringing is connted to a caller. the only thing that fixed the issue for a little while is to put a old vm back up on the system and the system works fine and has no problems for a while(few days to a week) then it comes back.

hear is the sw for all the cards i know theirs a 4.60b now any one know what it changes?
